Average Anthem Blue Cross Shield of Colorado member copay is 4 charges of $20, over the program's 12 weeks. If you have a high-deductible plan, your cost-share may be higher, but it won't be more than $325 per charge.
You won't be charged if you decide to not start the program during the intake call.
